Direct Admission For ESIC Paramedical Institution, Gulbarga

 Instructions to Applicants:

Applicants are to submit the applications through e-mail only with the subject “Application for Diploma in Medical Records Technology/ Diploma in Operation Theatre and Anaesthesia Technology”. The documents to be attached to the email are:

a) Scanned copy of duly filled & signed application form in Annexure-I (pdf format only, 200 to 300 kb in size and saved as “Application Form”)

b) Scanned Self attested copy of Class 12 /10+2/ PUC Marks Sheet (pdf format only, 200 to 300 kb in size and saved as “Marksheet”).

c) Scanned self attested copy of e-Pehchan certificate with the name of the candidate as dependent (pdf format only, 200-400 kb in size and saved as “Pehchan”).

d) Scanned copy of Caste Certificate -as applicable (pdf format only, 200 to 300 kb in size and saved as “Category certificate”).

e) Letter of declaration and undertaking to be submitted by the Insured Person (Annexure -2)

(II)Email ID for submission of application: Application shall be received at e-mail id: rectt-karnataka@esic.nic.in only. Application received by post or over any other email id or any other mode will not be accepted

(III) Last date& time for submission of application through e-mail: 22-10-2019 (06.00 PM). The date and time mentioned in the delivery inbox of the recipients e-mail id shall be treated as the date and time of receipt. Any issues arising out of internet / server connectivity will not be the responsibility of ESIC.

• Date of counselling for shortlisted candidates: Counselling for the short-listed candidates will be held on 25/10/2019 between 9.00 A.M. to 6.00 P.M. The shortlisted candidates will be displayed at www.esic.nic.in/admissions on 23/10/2019.
• Place for Counselling: Counselling will be held at ESIC Medical College and Post Graduate Institute of Medical Sciences and Research/ Model Hospital, Rajajinagar, Bangalore-560010
• Applications received without any of the requisite documents or received after the due date and time will be summarily rejected and no correspondence will be entertained in this regard.
• Scanned self attested copy of the e-Pehchan certificate showing the name of the candidate as dependent may be submitted. The e-Pehchan certificate should have the signature of the employer. On submission of e-Pehchan certificate eligibility under the Ward of IP quota will be subject to verification by ESIC.
• Counselling for the shortlisted applicants shall be conducted on 25-10-2019 between 09.00 AM to 06.00 PM at ESIC Medical College, Rajajinagar, Bangalore, Karnataka.

Applicants shall submit the following original documents for verification during Counselling:

• Duly stamped and signed original e-Pehchan certificate showing the name of the candidate as a dependent.
• Application and other documents like undertakings, etc., scanned and submitted through email to be submitted in original
• Class 12 /10+2/PUC / equivalent Certificate and Mark Sheet
• Class 10 / SSLC Certificate and Mark Sheet.
• Caste Certificate (as applicable, in the format prescribed by the Central Government).
• 2 Passport size Photos of the Candidate (same as the photo affixed on the application form).
• Aadhar Card or Voter ID Card or PAN Card or Passport or any other Govt. issued Photo Identity Card.

Note: In case of inability to produce original documents during counselling, the candidature of the applicant shall be treated as cancelled and no further correspondence shall be entertained in this regard.
